How We Work with Individuals, Families or Agencies

  1. Once a referral is received, the assigned support partner will contact the case manager and the family/youth/individual to introduce themselves and explain their role.

  2. Through genuine conversation, the support partner will begin establishing a relationship and building a rapport with their assigned family/youth/individual to learn about their culture, strengths and needs. The support partner will then help develop a plan to achieve the goals the family/youth/individual establishes for themselves.

  3. Weekly contact will occur through the recipient’s preferred method of communication. In time, we will partner to develop action steps to reach their established vision, and the support partner will operate in a way to best assist them.

  4. Ongoing communication will be maintained with the case manager who completed the referral or who is connected to the family/youth/individual (if applicable) to ensure a collaborative effort is made to achieve established goals.
